Skilled nursing care is a high level of medical care that must be provided by trained healthcare professionals. These services can be necessary over the short term for rehabilitation from an illness or injury, or they may be required over the long term for patients who need care on a frequent or around-the-clock basis due to a chronic medical condition.
Home care clinicians are trained and experienced in wound care management, the healing process, care products and devices such as Negative Pressure Wound Therapy (NPWT). The ultimate goal is to promote a faster healing with committed healthcare professionals and working as a team with the patient/caregiver and physician.
We understand how important it is for a patient to regain independence in a convenient and comfortable setting such as home. Trust our rehabilitation proffesionals to speed up patient’s recovery from an illness, injury or surgery. Our team consists of Physical Therapists, Ocuppational Therapists and Speech-Language Pathologists.
Medical Social Worker
Personal needs, limitations, barriers and/or safety issues can be the burden of an early recovery while admitted in home health care. Our Medical Social Workers play a vital role in the care team. This healthcare professional is able to educate and support patients/caregivers regarding community resources and other supportive options that would enable a continuity in the established plan of care.
Private Duty Nursing
We care about clients health that’s why we give Private duty nursing whether an RN (Registered Nurse) or LPN/LVN (Licensed Practical Nurse). We work one-on-one with individual clients where they need to, in the client’s home, or an institution, such as a hospital, nursing home or other such facility.
